I will personalize the monitor with a "to" name and a small message. (Example: "Happy Birthday" or "Happy Whateverholiday" or "Get Well Soon") The monitor will be sent to you in gif transparency. The adoption form is below the sample monitors. Only one monitor per adoption form, please. Make sure when you fill out the form and a confirmation page comes up, that you click on the "continue" button. If you don't, I will not receive your adoption request.
