I will personalize the Locket with a "to" name and a "from" name. The locket will be sent to you in gif transparency. Only one locket per adoption form, please. Make sure when you fill out the form and a confirmation page comes up, that you click on the "continue" button. If you don't, I will not receive your adoption request.
